Thanacare supports pediatric palliative care families by empowering and organizing

Enhance Family & Caregiver Experience
Improve family communication with Pediatric Palliative Care Coalition (PPCC) teams in order to relieve some of the burden of caregiving by matching the appropriate resources at the appropriate time. This increased alignment of knowledge and responsibilities enhances concordance between wishes and care received. Empowering the care circle to address their own well-being and needs as well as their children.

Improve Access to Resource
Lessen caregiver burnout and disorganization from delivering disparate resources by having a deeper understanding of a child’s needs and Goals of Care wishes thereby delivering the right resource and right communication in seconds.
Align Families, Kids, & Care TeamBy aligning communication and resources between PPCC and families and caregivers, the team supporting children with medical complex needs can reduce unwanted miscommunication, simplify tracking of information and resources, and effortlessly share vital health updates and detailed care instructions.

"Building a new program takes a community, and PPCC is excited to have Thanacare’s invaluable contribution to this important endeavor!"

Resource Matching Program Coordinator, PPCC

The Pediatric Palliative Care Coalition (PPCC) offers resources, education, and support for families of children with life-limiting conditions and the medical professionals who assist them. Through toolkits, webinars, advocacy, and more, PPCC strives to "Light the Way" in pediatric palliative and hospice care. PPCC's Resource Matching Program connects parents and caregivers of children with complex medical conditions and intellectual or developmental disabilities to reliable resources, easing the burden of navigating challenging times. The Firefly App powered by Thanacare helps parents organize medical records, track patient wishes and facilitate communication with the PPCC team, while allowing easy sharing of relevant resources.
